Stratis Health is a member of the Superior Health Quality Alliance (Superior Health), an eight-partner organization focused on improving care for Medicare beneficiaries. We collaborate with patients, clinicians, health care organizations, and communities across Illinois, Indiana, Michigan, Minnesota, Ohio, and Wisconsin to support the Centers for Medicare & Medicaid Services’ (CMS) quality improvement objectives.

Key Accomplishments

We have been participating in CMS’s quality improvement efforts since its inception, with the 12th Scope of Work (SOW) complete in 2024. Through this work, Superior Health achieved significant milestones, including:

  • 86% reduction in infection control citations among long-term care facilities
  • 58% relative improvement rate in beneficiaries with diabetes
  • 24% relative improvement rate in screening and diagnosing chronic kidney disease

13th SOW: A New Chapter

CMS has officially awarded Superior Health the 13th Scope of Work (SOW) as a Quality Innovation Network–Quality Improvement Organization (QIN-QIO). Superior Health was selected for its strong performance history and innovative approach to delivering person-centered, data-driven improvement strategies. This five-year contract (May 28, 2025–May 27, 2030) affirms our role as a trusted partner to CMS and expands our mission-driven work to reach more communities than ever before.

Superior Health will provide no-cost technical assistance to eligible providers, including support for chronic disease management, behavioral health integration, patient safety, care coordination, and health care quality — all aligned with CMS national goals.
