Minnesota health plans that offer publicly subsidized health care programs implement performance improvement projects (PIP) to help improve the health of public program members and to reduce disparities for low-income Minnesotans. The projects have covered a wide range of health topics identified as priorities for improvement, including preventive care, chronic illnesses management, and transitions in care. Each project typically runs three years and helps meet Minnesota Department of Human Services and Centers for Medicare & Medicaid Services (CMS) requirements.
Stratis Health provides facilitation and consultation to the health plans in developing, implementing, and evaluating each of the improvement initiatives. We also provide access to resources created through the PIP projects on our website. Resources include recorded webinar educational sessions, provider/care coordinator toolkits, and member education materials.
To be successful in improving care for members, health plans engage care coordinators and providers as appropriate to the PIP to help ensure that members receive preventive services, and support in managing chronic conditions and high-quality care as recommended for the topic.
