Care Coordination Toolkit
Community-based care coordination is a partnership among health care professionals, clinics and hospitals, specialists, pharmacists, mental health professionals, community services and other resources working together to provide patient-centered, coordinated care. Whether in the context of an accountable care organization (ACO), integrated health partnership (IHP), accountable communities for health (ACH), patient-centered medical home (PCMH), health care home (HCH) or other structure, the goals are the same: continually improve health care quality and patient experience while reducing overall costs.
Palliative Care Resource Center
Since 2008, Stratis Health has pioneered processes for establishing and supporting palliative care services in smaller, rural communities. Recognizing that existing best practices were designed for large hospitals, Stratis Health has addressed this gap by working with more than 40 rural communities across the country to build capacity to offer palliative care services.
Quality Improvement Basics
This QI Basics course is designed to equip professionals with the knowledge and tools to start quality improvement projects at their facilities. The course may be completed in sequence, or individual modules and tools may be used for stand-alone training and review.
Health Plan Performance Improvement Projects (PIPs)
Minnesota health plans that offer publicly subsidized health care programs implement performance improvement projects (PIP) to help improve the health of public program members and to reduce disparities for low-income Minnesotans. The projects have covered a wide range of health topics identified as priorities for improvement, including preventive care, chronic illnesses management, and transitions in care. Each project typically runs three years and helps meet Minnesota Department of Human Services and Centers for Medicare & Medicaid Services (CMS) requirements.
Root Cause Analysis Toolkit for Long-Term Care
Root Cause Analysis (RCA) is a problem solving method to investigate an actual or potential problem, incident or concern. A team looks beyond an immediate solution to understand the underlying cause(s) of the problem. Those causes are then changed to prevent the problem from happening again.
Emergency Department Transfer Communication
A National Quality Forum endorsed measure is available for hospitals to evaluate communication for transitions of care during emergency department (ED) transfers. This ED transfer communication measure is particularly relevant for critical access hospitals (CAHs) and is included in the Medicare Beneficiary Quality Improvement Project (MBQIP). MBQIP is a program of the Health Resources and Services Administration (HRSA) funded Office of Rural Health Policy’s (ORHP) Medicare Rural Hospital Flexibility Program (Flex).
