Multiple Chronic Conditions Resource Center

Packages

The Multiple Chronic Conditions Resource Center (MCCRC) is recognized by the U.S. Department of Health and Human Services as a premier resource for interdisciplinary health care professionals, patients, families, and policymakers. MCCRC has partnered with federal agencies, key thought leaders, legislators, and Stratis Health to promote best practices for the nation’s largest, fastest-growing, and costliest patient population – those with two or more chronic conditions. MCCRC offers a one-stop resource on current trends in clinical practice and health policy that support comprehensive patient-centered care for persons with multiple chronic conditions (MCC).

Quality Improvement Basics

This QI Basics course is designed to equip professionals with the knowledge and tools to start quality improvement projects at their facilities. The course may be completed sequentially, or individual modules and tools may be used for stand-alone training and review.

Center for Opioid Resources and Education

Packages

The Center for Opioid Resources and Education (CORE) supports clinical and non-clinical health care professionals in responding to the opioid public health emergency by bridging care gaps in disadvantaged, rural, and underserved populations, as part of the Stratis Health Opioid Addiction in Rural (SOAR) Extension for Community Healthcare Outcomes (ECHO) online learning series.

Service Recovery in Health Care

This brief four-part recorded training series equips health care professionals with the knowledge and tools to support service recovery training at their facilities. We encourage viewing the sessions in sequence, but individual sessions may be used for stand-alone training and review.

Care Coordination Toolkit

This toolkit strengthens the ability of professionals to increase care coordination and transitions.

Community-based care coordination is a partnership among health care professionals, clinics and hospitals, specialists, pharmacists, mental health professionals, community services and other resources working together to provide patient-centered, coordinated care. Whether in the context of an accountable care organization (ACO), integrated health partnership (IHP), accountable communities for health (ACH), patient-centered medical home (PCMH), health care home (HCH) or other structure, the goals are the same: continually improve health care quality and patient experience while reducing overall costs.

Rural Palliative Care Resource Center

Tools and resources to build rural community capacity to care for those with serious illness, including a Stratis Health-developed framework and toolkit for rural community-based palliative care program development reflecting foundational components, process development, and service implementation. This toolkit contains guidance and links to a comprehensive set of resources to support rural leaders in developing community-based palliative care services.

Root Cause Analysis Toolkit for Long-Term Care

Packages

This toolkit helps to examine and understand actual or potential problems, incidents, or concerns and anticipate them from happening again.

Root Cause Analysis (RCA) is a problem-solving method to investigate solutions to understand the underlying cause(s) of problems. Those causes are then changed to prevent the problem from happening again.

Health Plan Performance Improvement Projects (PIPs)

Packages

Minnesota health plans that offer publicly subsidized health care programs implement performance improvement projects (PIP) to help improve public program members’ health and reduce disparities for low-income Minnesotans. The library of resources highlights a wide range of health topics identified as priorities for improvement, including preventive care, chronic illnesses management, and transitions in care.
