Strong and healthy rural communities are vital to the American society and economy. Access to high-quality, local care delivery is an essential component of helping rural people and communities thrive.
Rural health care delivery is unique. Staff typically serve multiple roles, resources are generally limited, and available quality measures often do not appropriately account for the types of services provided or the volume of patients served.
Initiatives that successfully improve health outcomes in rural communities account for these unique circumstances. Through decades of experience working with a wide range of rural health care organizations and clinicians and engaging with rural partners locally and nationally, Stratis Health knows how to improve rural health effectively and efficiently.
“Stratis Health is exceptional to work with and has a great depth of knowledge in rural healthcare and providing technical assistance to CAHs.”– Laura Fischer, Illinois Critical Access Hospital Network (ICAHN)
“The Stratis Health team has been an excellent resource for us. They educated and helped us to understand the meaning of data trends and offered solutions to help us get back on track. I would recommend this group to any hospital wanting to gain an understanding of CMS and patient experience metrics and future trends.”– Chris Butler, COO/CNO, Adams Memorial Hospital |

Building bridges between health care and community:
- We support organizations in improving the health of their community by incorporating a wholistic approach to addressing health-related social needs (HRSN) and social determinants/drivers of health (SDOH). This includes—but is not limited to—implementing comprehensive screening and data collection processes, integrating community resources, and establishing closed-loop referral networks. We guide organizations in convening community partners to develop community solutions, offering structured processes and support to create collaborations that can expand reach and services, improve care coordination, and address community needs.
