Strengthening organizational capacity:
- We offer a variety of learning collaboratives and technical assistance offerings aimed at strengthening the organizational capacity of rural health care organizations. The resources we provide equip participants with the knowledge and best practices to address challenges and implement solutions designed to meet the needs of rural communities.
- The Stratis Health team has deep expertise in quality improvement and population health and can be available to support webinars or conference presentations on various topics relevant to rural health care organizations, such as small rural and critical access hospitals (CAHs) and rural health clinics (RHCs) that align with our transformation framework.
- We help organizations transition to value-based care and payment arrangements, leveraging evidence-based approaches to coordinating care, managing the health of populations, and activating patients, families, and communities to optimize outcomes.
- Through our community-based palliative care work, we assist rural communities in developing services and building skills to improve the quality of life and care for those with chronic diseases or life-limiting illnesses.
Building bridges between health care and community:
- We support organizations to advance health equity by incorporating a wholistic approach to addressing health-related social needs (HRSN) and social determinants/drivers of health (SDOH) – including but not limited to implementing comprehensive screening and data collection processes, integrating community resources, and establishing closed-loop referral networks.
- We guide organizations in convening community partners to develop community solutions, offering structured processes and support to develop collaborations that can expand reach and services, improve care coordination, and address community needs.
